Referrals for a guardian or conservator may be available from a county, nursing home, hospital, or community social worker; the attorney who brings the petition to court; the attorney appointed by the court to represent the person subject to guardian or conservator.

Biography
Allycia is the Lead Guardianship Line Specialist with the Center for Excellence in Supported Decision Making. Allycia has supported people with disabilities and older adults in a variety of ways—at home, work, and in their communities. For nearly a decade, Allycia has dedicated herself to education and sharing information on Supported Decision Making (SDM) options, and alternatives to guardianship.
Allycia believes everyone has the ability, with the right supports, to lead their own life. She knows each person can be an active, valued member of their community. Allycia has led presentations on SDM, ableism, community connections, future planning, and much more. She has served as a surrogate decision maker through appointment by a Power of Attorney and Health Care Directive.
Allycia is also a certified Health Care Facilitator through Honoring Choices Minnesota. In 2019, she produced and hosted the Focus on the Future podcast, an educational resource for caregivers of adults with disabilities. She has a Bachelor of Science in Vocational Rehabilitation, with a concentration in Psychiatric Rehabilitation.
